Welcome to the Healthy End of Life Planning (HELP) Brief Intervention Digital Training. This self-directed course is designed to fit your schedule, allowing you to learn at your own pace.

The training provides a simple, practical way to integrate social prescribing into your everyday practice. You will learn how to use the HELP framework to quickly connect people with life-limiting illness, and their carers, to the vital social and practical support they need—often within just a few minutes.

Through this course, you will:

• Learn to identify social support needs and enhance end-of-life experiences.
• Gain an overview of the HELP App and its resources.
• Build confidence in using HELP as an effective social prescription.

The training concludes with a short quiz to confirm your understanding. Upon completion, you will receive a digital certificate and have the option to connect with the HELP Team face-to-face.
