Welcome to the Healthy End of Life Planning (HELP) Brief Intervention Digital Training — designed for health professionals, aged care workers, community service providers, and volunteers. This training is designed to be self directed where you learn in your own time by viewing content and resources. You can stop at any point and return to it when you are next available. There is a short quiz at the end to ensure your understanding of the concepts.
Aim:
The HELP Brief Intervention provides a simple and practical way to integrate social prescribing into your everyday practice when supporting people living with life-limiting illness and their carers.
This digital training will equip you with the understanding, tools, and resources needed to help patients and clients access the social and practical support they need — often within just a few minutes of interaction.
Through this digital training, you will:
- Learn about social networks and how they can enhance end-of-life experiences
- Gain an overview of the HELP App
- Build confidence in using HELP as a social prescription with patients, families, and carers
Upon completion of the training, you will receive a digital certificate of completion and there will also be the option to connect with the HELP Team face to face.
